Jasper’s Memorial Hospital and Health Care Center and Deaconess Health Systems in Evansville have entered into an affiliation agreement that will work to enhance services and increase collaboration between the two healthcare providers and the counties they serve.
Affiliation allows the hospitals to work together on initiatives such as population health management, staff and physician education. It also helps provide transitions of care from the doctor’s office to medical specialists and advanced procedures.
Deaconess developed and manages the only comprehensive population health initiative in the region.
More than half of the hospitals in the tri-state area have partnered or affiliated with Deaconess. They include Methodist Hospital in Henderson, Ky., Gibson General Hospital in Princeton and Good Samaritan Hospital in Vincennes among others.
